Vyplní podnikateľ, ktorý má v deň ohlásenia živnosti zriadenú prevádzkareň
/ To be filled in by an entrepreneur who has established establishment on the day of notification of the trade.

Prevádzkarňou sa rozumie priestor, v ktorom sa prevádzkuje živnosť. Združenou prevádzkarňou sa rozumie trvalo zriadený priestor, v ktorom sa prevádzkuje výrobná živnosť na účel výroby  finálneho produktu v prípade,  ak pracovný, technický alebo technologický postup prác nadväzuje na seba alebo inak spolu súvisí.

Za súvisiace priestory sa považujú: sklady, stanovištia a odstavné plochy pre motorové vozidlá, garáže a priestory určené na zabezpečenie údržby a technickú kontrolu vozidiel, miesto podnikania, výstavné priestory a vzorkové predajne, priestory na prijímanie zákaziek alebo tovaru a na jeho vydávanie, kancelárie a pod.

 

An establishment shall mean premises where a trade is carried on. A combined establishment shall mean a permanently established premises where a manufacturing trade for the purpose of production of final product is carried on and in case that the work, technical or technological procedure are follow-up of each other or are mutually linked together otherwise for the purpose of production of the final product.

As the premises connected with pursuit of trade are deemed: premises reserved for storage, parking lots, car parks for motor vehicles, garages and premises reserved for maintenance and overhaul of motor vehicles, exhibition premises and premises for exposition of samples, premises for taking orders or goods and its delivery, offices and so on.

Vyplní podnikateľ, ktorý má v deň ohlásenia živnosti zriadenú prevádzkareň

Prevádzkarňou sa rozumie priestor, v ktorom sa prevádzkuje živnosť. Združenou prevádzkarňou sa rozumie trvalo zriadený priestor, v ktorom sa prevádzkuje výrobná živnosť na účel výroby  finálneho produktu v prípade,  ak pracovný, technický alebo technologický postup prác nadväzuje na seba alebo inak spolu súvisí.

Za súvisiace priestory sa považujú: sklady, stanovištia a odstavné plochy pre motorové vozidlá, garáže a priestory určené na zabezpečenie údržby a technickú kontrolu vozidiel, miesto podnikania, výstavné priestory a vzorkové predajne, priestory na prijímanie zákaziek alebo tovaru a na jeho vydávanie, kancelárie a pod.

Označte ak prikladáte oprávnenie užívať nehnuteľnosť. Oprávnenie užívať nehnuteľnosť prikladá fyzická osoba vtedy, ak adresa miesta podnikania je iná ako bydlisko fyzickej osoby. Zahraničná fyzická osoba  preukáže oprávnenie (doklad) užívať nehnuteľnosť (miesto podniku zahraničnej osoby alebo miesto činnosti organizačnej zložky podniku zahraničnej osoby)

Choose if you are attaching the document proving the beneficial ownership of the real estate. Document proving the  beneficial ownership of the real estate shall be attached if the address of the place of business differs from her/his residential address. Foreign person shall prove his/her beneficial ownership of the real estate (place of business of foreign person or place of business of an organisation unit of a foreign person)


Fyzická osoba predloží výpis  z registra trestov osôb, ktoré nie sú štátnymi občanmi Slovenskej republiky. Zahraničná fyzická osoba pripojí výpis z registra trestov osoby v postavení vedúceho podniku alebo vedúceho organizačnej zložky zahraničnej osoby, ktorá nie je štátnym občanom Slovenskej republiky. Povinnosť predložiť výpis z registra trestov majú aj osoby zodpovedných zástupcov, ktorí majú  bydlisko v Slovenskej republike  alebo iné oprávnenie na pobyt podľa osobitného predpisu, ale nie sú štátnymi občanmi Slovenskej republiky

Natural person shall attach extract from criminal record of persons who are not citizen of the Slovak Republic. Foreign natural person shall attach extract from criminal record of a manager of foreign person or manager of organisational unit of foreign person who is no citizen of the Slovak Republic. Authorised representatives who have permanent residence or permanent residence based upon permit pursuant to a special regulation but who are not citizens of the Slovak Republic shall submit the extract from criminal record.


Označte ak bol ustanovený zodpovedný zástupca a prikladáte jeho súhlas s ustanovením do funkcie pre viazanú alebo remeselnú živnosť. Súhlas zodpovedného zástupcu, musí byť podpísaný zodpovedným zástupcom buď vlastnoručne (ak je príloha naskenovaná) alebo podpísaný zaručeným elektronickým podpisom (KEP-om) zodpovedného zástupcu.
